Your passport · airline + flight number · date of arrival · accommodation name and address in Mauritius · an emergency contact · the list of countries you visited in the last six months.
Choose By air or By sea, then your country of residence, airline and flight number (tick "My flight is not listed" if it's missing), date of arrival (DD/MM/YYYY), purpose of travel, proposed length of stay, initial port of embarkation, and countries visited in the last six months.
